Novo Nordisk’s Stable-Margin Plan Makes Revenue Mix More Important

Citi Cuts Novo Nordisk Price Target to DKK 296 on Limited Margin Growth Outlook

Citi cut Novo Nordisk’s target to DKK 296 as management projected stable margins. Oral Wegovy may lift sales while R&D and capacity spending absorb leverage.

Novo Nordisk’s 2030 plan asks investors to accept a different growth profile: revenue can expand while the operating margin remains broadly stable. Citi responded by cutting its price target to DKK 296 from DKK 310 and reducing adjusted operating-profit estimates for 2028–2030 by 2% to 4%, while keeping a neutral rating.

The bank’s adjustment is directionally consistent with Novo’s September 21 Capital Markets Day. Management said it aims to maintain a broadly stable adjusted operating margin from a 2026 baseline. Novo explicitly described the 2030 ambitions as uncertain and not formal financial guidance.

Oral Wegovy changes volume and channel economics

The Wegovy pill can expand the obesity market by reaching patients who avoid injections and by simplifying distribution. It can also cannibalize injectable prescriptions or shift sales into channels with different rebates and gross margins. More patients do not automatically mean a higher operating margin.

Novo’s 2025 annual report said the January 2026 U.S. pill launch was already embedded in its outlook and depended on assumptions about market penetration, cannibalization of injectable treatments and channel mix. Those caveats explain why improving pill expectations can coexist with limited margin expansion.

Investment absorbs operating leverage

Novo spent DKK 60.1 billion on property, plant and equipment in 2025, up from DKK 47.2 billion in 2024, largely for active-ingredient and fill-finish capacity. That was a DKK 12.9 billion increase, or about 27%. Capacity spending can support future revenue and reduce shortages, but depreciation and startup costs weigh on profit and free cash flow before plants reach efficient utilization.

R&D is the other claim on leverage. Novo is advancing oral and injectable obesity candidates while competing with Eli Lilly and a wider field. A richer pipeline protects the franchise from semaglutide patent and competition risk, but successful clinical programs require sustained spending and may not generate revenue for years.

Stable margins can still produce profit growth

A flat margin on a larger revenue base generates higher operating profit. The valuation question is whether sales growth remains strong enough after price pressure, competition and product mix. If revenue grew 8% annually with an unchanged margin, operating profit would also grow roughly 8% before unusual items. If the margin slipped two percentage points, the same revenue growth would translate into slower profit growth; the exact effect depends on the starting margin.

This is why Citi’s target change is relatively modest. The concern is not that Novo has lost its obesity franchise overnight. It is that investors should not count on scale alone to expand profitability.

What would change the thesis

A successful oral launch with limited injection cannibalization, favorable reimbursement and high manufacturing utilization could lift revenue faster than expected without hurting margins. The bear case is stronger price competition, costly capacity, late-stage trial failures or a mix shift toward lower-margin channels.

The next useful disclosures are oral Wegovy prescriptions, realized pricing, gross margin, R&D intensity and capital-expenditure conversion into supply. Analyst targets will move again; those operating data will determine whether DKK 296 proves cautious or generous.
